Form SSA-7028, Notice to Third Party of Social Security Number Assignments is a form used to authorize the Social Security Administration (SSA) to release information about an individual to a third party for the purpose of preparing a wage or tax report or completing the individual's record. We often receive requests for a copy of Form SS-5 (Application for a Social Security Card) or a full Numident printout, a query that displays information extracted from the application for an original SSN card and subsequent replacement cards. If you are applying for disability benefits through the Social Security website, you will be prompted to fill out and sign the SSA-827 after you have submitted the rest of your application.
